Job Description


The Prescription for Hope Violence Intervention Specialist is responsible for providing comprehensive support services through wrap-around case management which assists patients, clients, and their families to find practical ways of overcoming social, emotional and environmental barriers to optimum functioning and attainment of health.  This position provides therapeutic intervention to individuals and families that ranges from assessment, crisis intervention, community resource referrals, patient/family advocacy, support, outreach and follow-up.


  • High school diploma with a minimum of two years of direct community experience and/or coordinating wrap around services
  • Bachelor's degree in social work, nursing, addiction, education, counseling, psychology, sociology, criminal justice or related field preferred
  • Valid Indiana driver's license that meets Eskenazi Health's driving policy requirements
  • Experience in a healthcare or community setting
  • Demonstration of knowledge of community resources
