The Health Connection Referral Coordinator answers phone calls within company guidelines, routes callers to appropriate destination, answers questions, and schedules appointments.


  • Receives and places inbound and outbound telephone scheduling patient calls
  • Establishes priorities, meets deadlines, follows written and verbal instructions
  • Answers phone calls within company guidelines, routes callers to appropriate destination, answers questions, and schedules appointments
  • Schedules patient referrals originating from the Wystech Referral system for the various Ambulatory Care Clinics and Specialty Clinics
  • Assists in hospital releases within a thirty (30) minute timeframe by scheduling patients during their hospital stay prior to discharge using laptop computer
  • Responds quickly to requests and takes action, giving attention to detail.  Explains delays 
  • Provides callers with individualized service and program information
  • Obtains and verifies medical record number for existing clients.  Refers all inquiries to the appropriate areas of services
  • Documents all referrals for medical, legal and statistical purposes
  • Verifies and updates demographic information
  • Validates insurance information based on the patient’s network
  • Performs data entry at the time of the referral
  • Participates in other call center related marketing activities
  • Keeps the work area cleaned and organized.  Maintains a sense of pride and “ownership” for the program
  • Serve as a patient advocate at all times
  • Complies and follows all department and hospital policies governing customer relations by displaying positive behaviors toward patients, fellow workers, departmental staff, physicians, nursing staff, visitors and all others they may come in contact with


  • Complies with HIPAA regulations and maintaining confidentiality of patient medical records, financial/billing, employee, and contact information
  • Participates in orientation/instruction of new personnel as required
  • Demonstrates professionalism in appearance and approach
  • Demonstrates responsibility for own professional growth
  • Participates in quality improvement processes as requested by director/manager
  • Utilizes interpersonal communication strategies/skills to achieve outcome/results with patients/families and others
  • Performs other job related duties as assigned


  • High school diploma or equivalent (GED)
  • Minimum of two years working in a healthcare setting
  • Internal computer training through the hospital educational departments
  • Must pass annual competency exams as required
