Job Description

JOB SUMMARY:

The CORE Social Care Coordinator will interact directly with patients and their family members and CORE Paramedic and CORE Social Worker in implementing care protocols and completing environmental and social assessment forms as required.  Essential job functions include, but may not be limited to:

  • Conducts a social and environmental needs assessment in patient home settings as well as  within other healthcare settings as appropriate
  • Manages data entry into the electronic medical record.  Clinical oversight of assessments gathered will be performed by the IEMS CORE Social Workers
  • Schedules patient and caregiver visits for CORE, assists with scheduling patient appointments with PCP as needed, etc.
  • Assists with patient transportation needs as identified
  • May assist the paramedic in capturing measurements and recording vital signs
  • May interview patients and record history, chief complaint, other findings
  • Provides community resource referrals, patient advocacy, support, outreach and follow-up as needs identified within the home setting.  Reports all needs identified to IEMS CORE Social Work staff members for assistance and/or follow up.  CORE Social Worker will have clinical oversight of the CORE Care Coordinator position. 
  • Provides education, training, and coaching to introduce or improve coping skills of patients and caregivers
  • Initiates advocacy/liaison role for patients in referrals to and from community agencies

REQUIREMENTS:

  • Minimum of 2 years of organized training in a healthcare field (i.e. medical assisting, nursing, social work, etc.) preferred.  Will accept work in a customer service focused position in lieu of healthcare field work.
  • Strong customer service skills required.
  • Associates degree in business or healthcare related field preferred
  • Valid Indiana driver's license that meets Indianapolis EMS's driving policy requirements
