The Community Health Worker is responsible for providing outreach and follow-up for prenatal clients under the direction of the Prenatal Care Coordinator Nurse and Prenatal Population Health Supervisor. The Community Health Worker helps to ensure continuity of care for prenatal/postpartum clients and their infants through home and community visits.


  • Minimum of high school diploma or GED required
  • Minimum of six months of demonstrated involvement with neighborhood organizations and/or community via paid or volunteer work preferred
  • Current Indiana driver's license, proof of automobile insurance, and a dependable car
  • Willingness to work with patients and their family members or other support persons in the health center, home settings, and community settings
