Eskenazi Health Midtown Community Mental Health, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. Eskenazi Health Midtown offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Eskenazi Health Midtown, Indiana’s first community mental health center has a philosophy of care that stresses strength-based, community centered treatment that utilizes the recovery model and welcomes all patients from children to seniors. 

Midtown’s mission is to Advocate, Care, Teach and Serve, with special emphasis on the vulnerable populations of Marion County, Indiana. Midtown is comprised of several clinics, outreach centers and community based services whose primary mission is to serve individuals with serious mental illness and chronic addictions, as well as seriously emotionally disturbed children and their families.

Eskenazi Health Midtown is seeking a Care Coordinator who will work with Therapists to provide services to patients while embracing, understanding and operating under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) and the spirit of motivational interviewing.

A successful Certified Occupational Therapist Assistant:

  • Treatment: Adapts evaluation and treatment appropriately based on the age and cultural background of each patient; evaluates, plans and implements appropriate and timely treatment based on patients’ physical and psychosocial rehab needs; provides education to the patient and family appropriate to age and cultural background of each patient; ensures treatments are creative and effective based on observation and peer report

  • Documentation: Accurately documents patient treatment per documentation standards and in accordance with departmental timetable (refer to Rehab Policies & Procedures); obtains appropriate signatures when required; ensures documentation justifies charges; ensures documentation reflects skilled therapeutic need; ensures documentation reflects functional improvement and progress towards established goals; ensures documentation is consistently completed in a timely manner and is available for review by other hospital staff including therapists; ensures documentation and patient charts are easy to locate

  • Productivity: Works at or above established productivity level; volunteers to take walk-in patients; volunteers to float to other teams as needed; adjusts schedule according to caseload; demonstrates effective time management skills

  • Administrative: Completes ancillary tasks that affect patient care; submits complete, legible and accurate charge sheets daily as reported by administrative staff and audits;  ensures e-learning and other assigned tasks are completed by the required due dates without reminders; ensures insurance paperwork is thorough and completed within 24 hours; ensures authorization requests are submitted when necessary without delaying patient care

  • Professional Communication/Collaboration: Demonstrates effective communication and teamwork with peers and other hospital staff to maximize effective/efficient patient care; maintains ongoing communication with the supervising therapist regarding progress toward goals; consults appropriately with physicians and other caregivers to obtain assistance or to provide advice regarding management of individual patients’ care; delegates routine aspects of care to PTAs /COTAs, Rehab Techs or others as appropriate; demonstrates effective verbal and written communication skills with physicians, patients, family members, and other professional staff; provides meaningful patient education materials when appropriate and completes available interdisciplinary communication forms; attends hospital rounds and care conferences; assists co-workers as needed; volunteers to work additional weekends/holidays

  • Performance Improvement: Actively embraces opportunities and ways of improving services, patient care and/or productivity by impacting cost, outcomes and/or patient satisfaction; provides value-added suggestions to improve services and performance; participates in committees and activities and takes an active role in improving processes for the hospital and department; finds new ways to provide services and utilize supplies to minimize costs; submits any appropriate PI data and reports in a timely manner without reminders; promotes change and assists coworkers to transition in a changing work environment

  • Student Education: In accordance with the Eskenazi Health mission, participates in the education of students in the field of therapy; effectively and efficiently manages the student’s learning experience; successfully passes clinical instruction competency; assures adequate orientation for student and student compliance with Policies & Procedures and HIPAA regulations; gives constructive feedback on regular basis; meets with student regularly; regularly observes students with various patient types and adjusts supervision levels accordingly; communicates appropriately with educational coordinator or school program coordinator; represents Eskenazi Health in a professional manner to the student

  • Clinical Competence: Demonstrates ongoing clinical competence and the initiative to stay current in the profession; successfully passes all competencies and serves as instructor/evaluator for orientations and designated competencies as appropriate; attends continuing education and pursues opportunities to improve clinically and professionally; attends department and hospital in-services and actively participates; seeks out knowledge and expertise from co-workers and shares their own knowledge with others; routinely treats a variety of patient diagnoses effectively; maintains membership and/or participates in respective professional organizations

  • Time and Priority Management: Minimizes time away from work and prioritizes multiple tasks to maximize time spent in direct patient care; reports to work on time and on a consistent basis; adheres to the hospital attendance policy guidelines; completes all assigned tasks and/or special projects on time without reminders; manages own schedule well, minimizing delays with patient care and scheduled appointments; demonstrates ability to triage patients to minimize functional decline and facilitate throughput

  • Licensure and Certification: Completes all other department licensure, certification and education records as required; completes TB, FIT, and CPR requirements on time and submits proof of completion to the immediate supervisor; completes all required annual education and e-learning lessons and submits to the immediate supervisor; maintains current state and national professional licenses; completes and provides documentation of external course records and requests

The qualified candidate will:

  • Licensed/Certified in the state of Indiana as a COTA or PTA
  • Graduate of approved curriculum with Associate’s Degree
