Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

Staff Nurse II - Resource Pool provides patient care which takes into account patient triage, response to care provided, patient needs, barriers, capabilities, limitations, and progress toward goals. The role of the nurse is to assess the patient, provide patient education, implementing plan of care utilizing nursing interventions, and crisis intervention. Triage is an interactive process between the nurse and patient that occurs over the telephone or face-to-face and involves identifying the nature and urgency of client healthcare needs and determining the appropriate disposition. The goal of telephone triage services is to direct the caller to the appropriate level of care or service in a safe and timely manner in combination with providing self-care advice and direction to the appropriate level of care based on protocol recommendations. The Staff Nurse II – Resource Pool directs the patient to the right level of care with the right provider in the right place at the right time. The Staff Nurse II – Resource Pool is responsible for patient outreach including panel management. May work at various work sites per operational need.  The Staff Nurse II – Resource Pool also provides health center support by performing non-patient facing tasks as needed, including In-basket management, referral follow up, prior authorizations and special project contributions.



Essential Functions and Responsibilities

•    Models Eskenazi Health Values and supports mission
•    Travels to Eskenazi Health Center sites as assigned to provide clinical support.
•    Timely in responding to patient/family and other team member needs and requests. 
•    Manages patient outreach by completing Epic inbaskets messages within the specified time frames.
•    Manages assigned referral work queues and prior authorizations.
•    Practice Eskenazi Model of Excellence (EME).
•    Enables colleagues to act through teamwork, encouragement and empowerment. 
•    Recognizes others; mentors team members 
•    Manages timelines/deadlines of team projects and site projects as assigned.
•    Participates in onboarding and signs off on staff competencies
•    Assist team members in growth by delegating tasks that are in alignment with their scope of practice and competence level and holding accountability
•    Recognize conflict and address it early; monitoring body language and subtle cues. 
•    An active listener to the patient, family, and staff member in conflict.
•    Works to seek a solution or a compromise that is good for both parties;  effective stewardship by assuring adequate resources are available and policies and procedures are followed

•    Manages triage of patients and families.  Identifies level of care needed and implement plan of action. 

•    Working with other nurses and medical assistants on the team provides nurse visits for patients assigned to the team, ensuring that members of the team provide care within their scope of practice.
•    Assumes responsibility and accountability for his/her actions.
•    Demonstrates a self-directed work effort aligned with team goals. 
•    Ensures that staff members have appropriate training and qualifications for the patient assignment. 
•    Works collaboratively with health care team in the care and care management of patients and their families.
•    Networks with other nurses to maintain current knowledge of issues and concerns.
•    Accountable for the safety and care of the patients/families.  
•    Communicates clearly and appropriately acting as liaison between the patient, family, and provider.
•    Consistently tracks, organizes, and prioritizes the workload with the ability to sort through routine and stat data while keeping both the provider and the patient informed in a timely manner.
•    Charts assessments, interventions, and patient response concisely, descriptively, and legibly in accordance with company policies.
•    Performs nursing procedures safely and efficiently.
•    Performs routine quality control monitoring and maintains appropriate records as assigned.
•    Assure that regulatory requirements are met. 
•    Alert to appropriate teams to potential admissions, patient/family needs for equipment in the home, therapies, referrals or procedures. 
•    Ability to change and manage based upon a patient’s or the team’s needs. 
•    Problem solver, using critical thinking skills in the care of the patients/families
 



Job Requirements

•    Registered Nurse (RN) with state licensure.
•    Valid Driver License to be able to travel within Eskenazi Health Centers.
•    Current CPR certification and TB certification (may be obtained within 6 months of hire).
•    A minimum of three years progressive professional experience as a nurse with at least 1 year in an ambulatory setting preferred.
•    Bilingual in English and Spanish preferred but not required; may be deemed necessary at certain locations. 
•    Fully proficient verbal and written communication skills. 
•    Experience with adult and/or pediatric IV and procedure skills preferred.
•    Knowledge of ICD-10/CPT coding.

•    Ability to perform triage assessment without face-to-face interaction.
