JOB SUMMARY: The Resource Coordinator uses intake information provided by the client to develop a client centered care plan. The Resource Coordinator arranges services for clients residing in the REC and seeks to connect them to appropriate community providers. The Resource Coordinator may conduct assessments with a focus on client empowerment.  



ESSENTIAL JOB FUNCTIONS: • Reach out to community organizations to find partners for the COAP grant • Collect W-9s for recovery homes that wish to partner with the Reuben Engagement Center to serve clients on the COAP grant • Track COAP grant related data in REDCAP • Identify individuals to be served on the COAP grant • Assist with client follow-up related to COAP grant • Assist with writing discharge case plans for COAP grant participants • Coordinates referrals up to 30 clients who are in the Reuben Engagement Center (REC), with a key focus on client empowerment and assuring that client referrals and coordinated services are based on client choice • Reviews assessments, service plans, progress notes and other records and documentation for accuracy; schedules daily tasks for intake interviews and individual assessments • Develops client service and discharge plans collaboratively with clients and other REC Staff, to include medical and management staff • Confirms proposed client placement with approved providers; acts as a liaison in the 'warm handoff”' to approved providers based on the care plan; may include arranging visits and transportation • Coordinates client placement in other needed services; may include working with psychiatric, medical, dental, job training, workshops and other identified community services as needed; responsible for periodic monitoring of client interaction with community provider post-discharge • Maintains positive relationships in a professional manner based on the mission of the REC and with the best interest of the client in mind • Responsible for data collection and data entry including coordination with information from community partners; completes and prepares reports as requested • Provides input on an ongoing basis to develop and improve program servicing and delivery • Keeps current by reading memos, directives, meeting minutes and revised policies and procedures as posted • Participates in education and training as required for the position including but not limited to any continuing education and basic medical (CPR, basic First Aid, etc.) • Other grant related duties as assigned

ASSOCIATED JOB DUTIES: • Performs all other job-related duties as assigned



JOB REQUIREMENTS: • Associate's degree in a relevant field; three years of experience working with substance abusing adults and a state recognized certification may be substituted for the degree • Valid Indiana driver's license and current automobile insurance policy as required by the state of Indiana • Certification as a trained Assessor/Counselor/Case Manager for chemical dependency is preferred  



KNOWLEDGE, SKILLS, & ABILITIES: • Experience with and personal understanding of homeless, alcohol/substance abusing persons, dual diagnosed, criminal justice, veteran, and/or culturally diverse population • Experience with and ability to use standard office equipment including computers, telephone/fax, copier/printer and scanner; proficiency in Microsoft Office applications • Ability to establish and set priorities, providing and responding to time sensitive information and requests as required • Ability to plan and accomplish assignments in a timely manner with a high emphasis on placing information into the various data formats, and the individual personnel filing system • Ability to communicate effectively in verbal and written formats; possesses active listening skills • Knowledge of relevant policies and procedures; ability to apply appropriately

SPECIAL EFFORT REQUIRED: • Ability to lift up to 20 lbs.



WORKING ENVIRONMENT: • Overnight clinical settings with the potential for physical interaction with clients
