Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 315-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.



JOB SUMMARY:

WeCare Plus is a program designed to reduce the risk factors for infant mortality through a combination of two way SMS messaging (text messages) and health coaching.  The five targeted factors include:        

 

Decrease in: 1) Smoking, 2) Obesity, 3) Mental health issues

 

Increase in:  1) Safe sleep practices, 2) Breastfeeding



ESSENTIAL JOB FUNCTIONS:

  • Proactively contributes to Eskenazi Health mission:  Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County.  Models Eskenazi Health values
  • Provides outreach and follow-up for prenatal clients under the direction of the Prenatal Population Health Coordinator Nurse & Prenatal Population Health Supervisor
  • Ensures continuity of care for prenatal/postpartum clients and their infants through home and community visits
  • Develops supportive and therapeutic relationships with clients and their families
  • Provides home visits to clients throughout pregnancy and early infancy to provide support, encouragement and guidance
  • Collaborates with Prenatal Population Health Coordinator Nurse through review of individualized care plans, sharing information concerning clients, reporting progress of families and community outreach results
  • Carries a caseload of at least 50 clients
  • Accurately documents home/telephone visit outcomes in a timely manner
  • Use motivational interviewing to encourage new or expectant moms and dads to modify their behaviors
  • Use text messaging service to educate and guide program participants through various health discussions
  • Recommend resources for new or expectant moms and dads in order to meet the basic 5 needs of the WeCare Plus participants: housing, food, clothing, transportation, utilities
  • Assists women in obtaining early prenatal care, arranging transportation, and making appointments for services
  • Accompanies clients to various service appointments as needed
  • Provides education concerning substance use/abuse and promotes cessation
  • Advocates for client needs
  • Promotes healthy birth outcomes by focusing on preventing pre-term labor, infections, and conditions that could impact pregnancy
  • Encourages preventive child health care such as immunizations and medical appointments
  • Maintains smooth working relationship and open communication with referral sites
  • Helps decrease child abuse and neglect by improving and enhancing parenting skills



ASSOCIATED JOB DUTIES:

  • Performs all other job related duties as assigned
  • Assists in the completion of application forms such as Hoosier Healthwise, Medicaid, Food Stamps, etc.
  • Assists with appropriate referrals to community agencies
  • Participates in staff and team meetings
  • Attends in-services/training to increase job knowledge



JOB REQUIREMENTS:

  • Minimum of high school diploma or GED required
  • Minimum of six months of demonstrated involvement with neighborhood organizations and/or community via paid or volunteer work preferred
  • Current Indiana driver’s license, proof of automobile insurance, and a dependable car
  • Willingness to work with patients and their family members or other support persons in the health center, home settings, and community settings
